    Professor Roger Seheult, MD discusses new data on dropping death rates for COVID-19 (and possible causes) and a new study on NAC (N-acetyl cysteine) in hospitalized patients with COVID 19. In the small study, there was a significant downward trend of markers of inflammation (C-reactive protein, bilirubin, and ferritin with each NAC infusion.     N-Acetylcysteine (NAC) is an over the counter supplement that has been utilized by clinicians for years to treat acetaminophen (Tylenol) overdose, as a mucolytic, and for other medical problems. There is compelling research that NAC may also reduce the severity of viral infections like influenza, and perhaps reduce the risk of blood clots (thromboembolism) as well. Could NAC be effective in COVID-19? Dr. Seheult illustrates these ideas and discusses the peer-reviewed data involved.

    We recorded this video in response to your comments asking what Dr. Seheult's daily regimen is - to optimize his immune system and minimize COVID-19 risk. In the absence of good clinical data that is specific to SARS-CoV-2, Dr. Seheult discusses supplements, sleep, and his daily disinfection routine when removing PPE, getting in his car, and returning home from the hospital. We look forward to more randomized, blinded, placebo-control studies in the future that will build upon the current body of evidence as it relates to immunity and specifically coronaviruses such as SARS-CoV-2.

  • N-Acetylcysteine And COVID-19: How Does NAC Work/Efficacy In 10 Patients With Severe COVID-19

    In this video we will discuss N-Acetylcysteine or NAC. We will talk about NAC and its role in glutathione synthesis, as well as its utility for free oxygen radicals and superoxide species. We will transition in to how this mechanism may be efficacious in COVID-19. After that, we will dive into a case series where 10 patients were treated for severe COVID-19 with NAC. They initially started it in the first patient as a treatment for hydroxychloroquine-induced hemolysis in the setting of G6PD Deficiency. They observed that this patient's inflammatory markers decreased after starting NAC, went back up after stopping NAC, and decreased again after re-starting NAC. After observing this they then went and gave 9 additional patients intravenous NAC and observed their clinical progression. All of these patients were critically ill on VA ECMO. At the time of study completion, 8 of the 10 had been discharged and the other 2 were still hospitalized. What does this mean? Check out the video for all the details!

Roger Seheult, MD of discusses new research from the Lancet on coagulopathies (clotting or bleeding problems) in COVID-19 and explains how reactive oxygen species (ROS) may set the stage for clotting.

    Dr. Seheult also illustrates how the supplement N-acetylcysteine (NAC) may be beneficial to reduce excess ROS and prevent blood clots. Sloan Kettering is performing a clinical trial that studies N-acetylcysteine (NAC) with COVID 19 infection. (This video was recorded July 3, 2020).

    No mRNA vaccine has ever been approved by the FDA, but Moderna and Pfizer/BioNTech hope to receive emergency approval based on preliminary data from their ongoing vaccine trials.

    Moderna's two-dose vaccine regimen does not require special refrigeration (the Pfizer vaccine is supposed to be stored at -70 Celcius) but a variety of questions remain:
    Will the vaccine prevent transmission and asymptomatic spread?
    How long will immunity last?

    This two-dose vaccine regimen has limitations including the need for -70 C refrigeration and many questions remain:
    Will the vaccine prevent transmission and asymptomatic spread?
    How long will immunity last?
    Will “90% effective” hold up to peer-review and additional data when it is gathered?

  • COVID 19 Update Update 106: Comorbidities and Excess Deaths; Bradykinin and Coronavirus

    Excess deaths are deaths above what is historically expected during a period of time. Dr. Seheult of explains how excess deaths are a key tool in understanding the impact of an epidemic - whether it is caused by influenza, COVID 19, or another virus - and helps dispel ideas of inaccurate death certificates by going through the steps of filling a death certificate out.

    An interesting hypothesis has emerged (that was initially formulated on a supercomputer) about the possible role of bradykinin in COVID-19 morbidity and mortality. The researchers surmise that bradykinin storm may have a greater impact than a cytokine storm as others have suggested (This video was recorded September 3, 2020).

    Convalescent Plasma Treatment for COVID 19 has been widespread in the US. Following infection, many patients have donated their plasma containing SARS-CoV-2 antibodies to try to help patients with severe symptoms fight COVID-19. The big news today is that the emergency approval from the FDA for the use of plasma treatment is now on hold based on a lack of robust data that demonstrates efficacy.     This video will discuss why zinc, quercetin, vitamin C and D, and n-acetyl cysteine and melatonin have been suggested as potential prophylaxis and even treatment for early symptoms of COVID.

    Vitamin D and COVID


    Right now, for early COVID infections, it is mostly supportive therapies just like for any common colds: bed rest, drinking plenty of fluids, aspirin and acetaminophen, cough, and cold syrups.

    Zinc is an essential mineral that has been thought to improve immune function, cardiovascular health, control blood sugar, slow macular degeneration, and fight acne. Zinc is found in oysters, chicken, red meat, and fortified breakfast cereals. While whole food sources are the ways to get zinc, there is evidence that supplemental zinc can aid your immune system.

    Research has shown that oral zinc formulations may shorten the duration of common cold symptoms, especially due to rhinovirus directly inhibiting viral replication. In addition, your body requires zinc to develop and activate T-cells, and decreased zinc levels have been associated with depressed immune function.

    Quercetin is a plant pigment that is believed to have anti-carcinogenic, anti-inflammatory, antiviral activities, and inhibition of pro-inflammatory mediators. It is also an ionophore meaning it creates a channel in the cell to allow zinc in.

    Vitamin C exerts its antiviral properties by supporting lymphocyte activity, increasing the production of interferon-α, modulating cytokines, reducing inflammation,

    Glutathione also acts as a free radical scavenger in the body. It has been suggested that elderly adults have decreased glutathione levels, leaving them susceptible to COVID inflammation oxidative stress.

    N-acetylcysteine (NAC) is a precursor of glutathione, described as “the master antioxidant,” Some studies show that NAC can increase glutathione levels. For example, an FDA-approved inhaled version of NAC called Mucomyst, which we used to decrease mucus's viscosity in patients in the hospital.

    Melatonin exerts regulatory actions on the immune system. It directly enhances the immune response by improving the proliferation and maturation of natural killing cells, T and B lymphocytes, granulocytes, and monocytes in the bone marrow and other tissues.

    Besides sleep disorders, melatonin has been used to treat delirium, atherosclerosis, respiratory diseases, and viral infections. In regards to viral infections, melatonin has anti-inflammatory, anti-oxidant, and immunomodulating properties.

  • Death Counts from COVID-19 | Excess Mortality Explained

    Let's use science and data to explore trends in excess mortality. What is the real Covid death total?

    As of October 27, there have been 1.17 million deaths attributed to COVID-19 across the world. Over 230,000 of those deaths have been recorded in the US. If these numbers are true, it would place COVID-19 among the top causes of death worldwide, among road injuries and diabetes, and at number 3 in the US, only lower than heart disease and cancer. This can be staggering and hard to believe. Often the question is asked: what is the real Covid death total? How many global deaths have been caused by the #coronavirus? What really is the Covid mortality? To answer that, we’re going to look at what’s called excess mortality. Throughout the video I’ll answer three question: What is excess mortality? What does the data show in 2020? And what can we learn from the data?

    We will look at excess mortality data from all over the world, including Mexico, Brazil, and many European countries, as experienced throughout the #Covid-19 pandemic.

    Excess mortality is the number of deaths from all causes that are above and beyond what we would have expected to see under ‘normal’ conditions. The formal term for total deaths is all-cause #mortality. All-cause mortality just means total number of deaths by any cause. We don’t have to classify or categorize deaths by their cause. It’s just a total count of deaths. And when we say normal level, it means we are comparing against the average number of deaths, usually looking at the most recent 5 years.

    In much of North and South America, deaths have exceeded normal levels throughout the entire #pandemic and have not returned to normal levels. Brazil, Mexico, and Peru, for example have had deaths 19, 61, and 141% higher than normal and excess mortality has been observed all year. Many countries in Europe, however, experienced a spike in excess mortality in March and April, and then deaths have returned to normal in the summer months. In some countries, including Iceland, South Korea, and Thailand, they have not observed any excess mortality at all.

    Unlike COVID-19 death counts, estimating excess all-cause mortality using a standard approach across countries provides a powerful tool to rapidly obtain unbiased estimates of the COVID-19 mortality burden.

    Topics explored in this video include deaths in 2020 vs average, deaths in 2020 vs 2019 and other previous years in the US and other countries, and will use tools such as the provisional death count provided by the CDC.

    The relationship between antibodies for the novel coronavirus (SARS-CoV-2) and immunity, vaccine efficacy, and spread of COVID-19 has received significant attention. Dr. Seheult is back to illustrate why T cells have received less attention than antibodies, but may be more important to long term immunity from COVID-19. Dr. Seheult also discusses how previous infections with common coronaviruses may provide some immunity to SARS-CoV-2, and the hypothesis that reducing the inoculum (initial dose) of SARS-CoV-2 might result in less symptom severity (This video was recorded August 10, 2020).

    “Extra’s” Billy Bush sat down with Dr. Armand Dorian, who talked about the recent dip in deaths from COVID-19.

    Dr. Dorian explained it is likely because “we are treating it better… The strain that is out now is probably not as deadly… The people who are getting infected are the younger crowd, who are more apt to not get significantly ill, so that skews the numbers.”

    Dr. Dorian also talked about the timeline for a coronavirus vaccine, and the possibility that President Trump could fire Dr. Anthony Fauci.
