The world's second-worst affected country by the pandemic had only Friday decided to resume international passenger flights from countries deemed at risk of the coronavirus, while ordering tightened border screening. But after the World Health Organization (WHO) declared the new variant to be of concern, PM Modi highlighted the need for monitoring all international arrivals, their testing as per guidelines, with a specific focus on countries identified 'at risk', the government said in a statement after he met with officials to review the COVID-19 and vaccination situation.     In a 20-minute interview to Karan Thapar for The Wire, Prof. Ravindra Gupta, Professor of Clinical Microbiology at the Cambridge Institute for Therapeutic Immunology and Infectious Diseases, first explained his research which establishes why Omicron leads to mild disease compared to other variants of Coronavirus. The first reason is to do with Omicron’s cleavage efficiency which means its ability to enter the cells of the body is restricted and not as effective as earlier variants. The second reason is to do with the way Omicron fuses with the cells of the human body. Unlike Delta, it doesn’t lead to the creation of giant infected cells called Syncytia.

    Both these reasons explain why the disease Omicron causes is mild and that, in turn, explains the data we have from South Africa as well as the UK which shows that though cases of Omicron are exploding the incidence of severe disease as well as hospital occupancy is limited and a lot lower than it was during the Delta wave.

  • Omicron - Incubation And Contagious Period

    Omicron continues to spread and we are starting to learn more and more about how this variant differs from previous variants. In this video we discuss the incubation period of the virus, or how long after you are exposed do you typically develop symptoms. We discuss how this differs from previous variants with some theories as to why. We also talk about how long you may be contagious and touch on the new CDC guidelines.

    The median incubation period for Omicron seems to around 3 days per the more recent data out of Norway, Nebraska, and the UK. With this being said, 0-8 days has been reported, so we cannot completely count on the 3 day number. This seems to be similar in vaccinated, unvaccinated, and previously infected although the data on unvaccinated is limited.



    This faster incubation period and increased transmission may be because Omicron seems to replicate much much faster in the upper airways than Delta does.



    The CDC shortened their quarantine period to 5 days with another 5 days masked in public. This seems to be an attempt to get people back to work more quickly, rather than a statement that you are only contagious for 5 days. Data suggests you are most contagious 1-2 days before symptoms and 2-3 days after symptoms, but may extend beyond this.

  • What do we know about Omicron till now?

    Another year, and yet another variant. Governments, citizens and scientists are treading with caution before revealing any new information about Omicron, a new variant of concern of the COVID-19 virus.

    This time last year, the Delta variant had arrived. It was first detected in India and then spread across the world to become the most dominant strain subsequently being the primary factor behind the deadly second wave.

    South Africa — where Omicron was first isolated on 24 November — has already declared its fourth wave as county after county begins recording a dangerous spike in cases. The new variant has been detected in at least 30 countries, with India reporting its first two cases on 2nd December 2021. .

    What do we know about Omicron till now?

    The Omicron variant has 50 mutations overall, with 32 mutations on the spike protein alone. The spike protein – which forms protruding knobs on the outside of the virus – helps it adhere to cells to gain entry. It is also the protein that all three vaccines currently available in the US use to induce protective antibodies.

    In comparison, the Delta variant has nine mutations. The larger number of mutations in the omicron variant may mean that it could be more transmissible and/or better at evading immune protection.

    Since its detection, South Africa’s daily caseload has gone up from 200 in mid-November to 11,500 on 3 December. The Gauteng province, where the infection was first found, has reported a 360 per cent rise in cases between 21 and 28 November 2021 with a 300% increase in hospitalisation and 17.65% increase in deaths in just 28 days.

    Omicron has taken over Delta to become the most dominant in South Africa. Over 70 per cent of all the virus genomes it sequenced in November have been of the new variant, as per South Africa’s National Institute for Communicable Diseases (NCID).

    The South African health agency has gone on record saying that people infected by other variants like Delta in the past are getting re-infected with Omicron. While its behaviour and characteristics are yet to be clinically studied, preliminary reports reveal Omicron may have the ability to evade immunity, developed either through natural infection or vaccination.

    Vaccines are still believed to be protecting against severe infections or death caused by COVID-19. However, work on variant-specific vaccines by leading manufacturers including Pfizer and Moderna is already underway.

    Debates surrounding the waning immunity concept have been gaining momentum over the past few weeks. Several studies, yet to be peer-reviewed, have shown a decline in antibodies after about five months of the second dose, thus causing breakthrough infections. However, immunologists have repeatedly cautioned against an overreaction to such concerns.

    Currently, immunity is tested through antibody tests. This metric remains slightly inaccurate because the body has other reserves to fight off an infection — the memory B and memory T cells. This part of our immune system is responsible for mustering a response against severe disease or death. A memory B cell produces antibodies, while a memory T cell destroys infected cells once activated. The latter remains in the body for a long period of time in a dormant state.

    With regard to the COVID-19 vaccines, it is as yet unknown at what rate our memory B and memory T cells decline. Their lifespan changes based on the virus. For instance, a vaccine for measles gives lifelong protection, while regular immunisation is needed for protection against the influenza virus every season.

    Barring its potential to evade immunity, the Omicron variant raises concerns because of how infectious it may be. The R value — defined as how many people an infected person can spread the virus to — for the Delta variant is between 6 and 7. The current R value for Omicron is 2, but it was 1 in September when the Delta variant was dominant.

    Tom Wenseleers, an evolutionary biologist at KU-Leuven in Belgium, was quoted as saying in the journal Nature: “Omicron has the potential to spread much faster and infect vastly more people than Delta. It can infect 3 to 6 times as many people as Delta, over the same time period,” calling it a huge advantage for the virus.

    The Omicron variant is spreading fast.
    Health experts believe it will soon become the dominant strain worldwide.
    There are concerns the two dose vaccine regimens may not be enough for protection.
    Han Seong-woo has the details.
    A joint study conducted in the United States has found that all three U.S.-authorized COVID-19 vaccines appear to be significantly less effective against the newly detected Omicron variant.
    Reuters reported on Tuesday that researchers at Massachusetts General Hospital, Harvard University and MIT had tested blood samples of fully vaccinated Pfizer, Moderna and Janssen recipients against an artificially engineered pseudovirus resembling the new strain.
    According to the experiment, which hasn't yet been peer reviewed, antibody neutralization of the variant from two doses of Moderna or Pfizer or a single dose of Janssen was (quote) low to absent.
    However, blood from recent booster shot recipients showed potent neutralization.
    The finding falls in line with a recent University of Oxford study suggesting the two-dose Pfizer and AstraZeneca regimens do not induce enough neutralizing antibodies against the new variant.
    BioNTech and Pfizer also said last week that although a three-shot course of their vaccine was able to neutralize Omicron in a lab test, two doses resulted in a considerably weaker effect.
    Alarmed at the risks Omicron poses, the World Health Organization on Tuesday urged nations to step up their prevention measures.

    Seventy-seven countries have now reported cases of Omicron, and the reality is that Omicron is probably in most countries, even if it hasn't been detected yet. Omicron is spreading at a rate we have not seen with any previous variant.

    The WHO chief also voiced concerns that a global rush to procure booster shots may further worsen vaccine inequity between developed and developing countries.
    Meanwhile, Director of the United States' National Institute of Allergy and Infectious Diseases Anthony Fauci said in an interview on Tuesday that the Omicron variant will likely become the dominant strain in the U.S.
    More research is underway but experts in South Korea say Asia will likely follow a similar trend.

    If you look at the situation in other countries, it's proving to be more contagious than the Delta variant. So, I think it's only a matter of time before Omicron becomes the dominant strain in South Korea as well.

    We begin with COVID-19 situation in South Korea.
    As the daily caseload continues to rise, health authorities have slammed the brakes on plans for a gradual return to normal... with new stricter distancing and antivirus measures taking effect starting on Saturday.

    For more on this and other COVID-19 related news... joining us live in studio, we have our Choi Won-jong. Welcome.

    Morning.

    Won-jong, what's today's forecast for the number of new cases?

    Yes, Friday's expected caseload will be slightly down compared to the previous day... hovering around the 7-thousand mark.
    Up to 9 PM yesterday... a total of 5-thousand 901 new infections were reported.
    This is 6-hundred 63 LESS than the same period from the previous day.
    More than 71-percent of those were reported in the Greater Seoul area.
    HOWEVER, the concerning part is that the number of daily infections in SEOUL continues to hit record-highs.
    Up to 9 PM on Thursday... 25-hundred and 4 were from SEOUL alone... which is the highest figure reported so far for the same period of time.
    We have to see if numbers will get higher in next couple hours or so.

    Won-jong, yesterday, the South Korean government announced much stricter social distancing measures to stop further spread. Tell us more about this announcement.

    Yes, after 45 days of its exit strategy... the government announced new social restrictions.
    Let's take a rundown of the new measures.
    Under the new rules, which kick in on Saturday, a maximum of FOUR people can meet nationwide,... no matter where they meet, and all of them have to be fully vaccinated.
    The unvaccinated... are not allowed to meet in groups,... so eating alone or food delivery is their only options.
    There's also a lower cap on events like weddings and funerals... where up to 49 people are allowed regardless of their vaccination status.
    However, 299 people can attend if they are all fully vaccinated.
    For businesses such as restaurants, cafes, singing rooms, saunas and gyms... they can stay open until 9 PM,... while movie theaters, concert halls, and PC rooms can operate until 10 PM.
    No limits have been imposed... on private academies.
    Also, vaccine passes will be required for visitors... at all conferences and exhibitions.

    The trend in terms of the daily caseload is on a strong incline, there's no denying that. Are there any other measures besides stricter social distancing?

    Yes, there are.
    South Korea's top health official held a briefing Thursday afternoon... stressing the importance of getting vaccinated as soon as possible.
    It's because more than 30-percent of daily infections are reported from people aged above 60.
    The authorities said... most of the severely-ill cases and deaths are also coming from the same age bracket.
    People aged over 60 make up around 80-percent of severely-ill cases and 95-percent of deaths.
    That's why authorities have also urged the public to get additional shots to boost waning immunity.
    And the booster shots are more than 70-percent effective fighting against Omicron variant.

    According to recent data released by the UK government, Pfizer's additional shots after getting two doses from AstraZeneca and Pfizer showed 90-percent efficacy against the Delta variant and 70 to 75 percent efficacy against Omicron.

    Regarding potential side effects from booster shots...the expert gave reassurances about the safety of the booster shots.
    Take a listen to what the expert had to say.

    Comparing side effects when getting first, second, and third shots... the occurence was similiar or even slightly lower. You don't need to think that there is a higher risk of side effects after getting the third shot.

    The authorities added that getting booster shots are a necessary part of vaccination... and will focus on administering jabs to the elderly in December.

    Won-jong, before you go... what's the current Omicron situation across the world?

    Well.. let's start with the French government's response to the spread of Omicron.
    Authorities in France on Thursday imposed travel restrictions on the UK... banning non-essential travel to and from Britain starting this weekend.
    Only French citizens and travelers from EU countries are allowed to enter the country.

    This came as the UK government saw record-high daily infections of more than 80-thousand.
    And, more than 51-percent of the total caseload in London is reported to be the Omicron variant.
    The UK health authorities predicted that one Covid patient with Omicron can spread the virus to three to five people... which shows it is more transmissible than Delta.

Tanzania returnee who tested positive was admitted to the Lok Nayak Jai Prakash Narayan (LNJP) Hospital on December 05. LNJP Medical Director Dr Suresh Kumar on December 05 said, “The Tanzania returnee experienced sore throat, weakness and body ache. His contact tracing is being done. He had taken both doses of COVID-19 vaccine due to which he had mild symptoms. All 17 COVID-19 patients admitted at LNJP are in stable condition. Most patients are asymptomatic. 4 more people have been admitted today. A special team has been constituted to attend to Omicron cases.” (JA)
