  • Acute Respiratory Distress Syndrome : Causes, Symptoms, Treatment | Why COVID is So Deadly

    Acute Respiratory Distress Syndrome (ARDS) is a clinical condition that is characterized by respiratory failure. ARDS can be caused by sepsis, COVID infection, pneumonia, chest trauma, generalized trauma, aspiration of gastric contents, TRALI (transfusion related acute lung injury), and more.

    No matter the cause, ARDS has characteristics: pulmonary edema, fibrotic changes in the lungs (formation of a hyaline membrane), and pulmonary hypertension.

    The hallmark sign of ARDS is hypoxemia despite increasing FiO2.

    Treatment of ARDS focuses on treating the underlying cause and increasing oxygenation, as well as hemodynamics stability.
    Prone positioning is a strategy that can be helpful to increase oxygenation.

    Despite treatment, however, the mortality of ARDS is 50%.

    Acute respiratory distress syndrome (ARDS) is a severe lung condition that occurs when fluid leaked from the smallest blood vessels in the lungs fills into the tiny air sacs. In normal conditions, these vessels are protected by a membrane but the severe illness can cause damage to the membrane leading to ARDS. Acute respiratory distress syndrome (ARDS) is one of the common clinical manifestations of severe COVID-19. Mr. Surendra was referred to Yashoda Hospitals from Nanded in Maharashtra, for treatment of COVID-19 ARDS (Acute Respiratory Distress Syndrome). He was in an extremely critical situation at the time of admission with severe respiratory distress.     Covid 19 and Acute Respiratory Distress Syndrome (ARDS) Diagnosis and Treatment
    Dr. Xuanha White, DO talks about COVID-19 Pneumonia and ARDS.

    ARDS, or Acute Respiratory Distress Syndrome, refers to a type of lung damage that can be caused by illness, trauma, or as a result of certain medical procedures. It's a potentially fatal condition in which the lungs suffer a serious injury that hurts their ability to provide oxygen to the body's organs. The condition causes fluid to build up in the lungs, which reduces blood oxygen levels to dangerous levels. ARDS is a serious medical issue.

    ARDS is always caused by an injury to the lungs, whether from illness or trauma. It can affect people of any age. Approximately 190,000 Americans are diagnosed with ARDS every year. Patients with ARDS need supplemental oxygen and are usually on a mechanical ventilator to breathe. While there is no cure for ARDS, it isn't always fatal. 60% to 75% of people with ARDS survive with treatment.

    We have a substantial amount of experience supporting people with acute respiratory distress syndrome, says Dr. Xuana White, a Pulmonologist and critical care specialist with Integra Health in Temecula, CA. But it's still a serious issue. Even though we can support the body to let it recover, the lungs still have to start the recovery process. ARDS has no magic bullet treatment.”

    How does COVID-19 relate to ARDS?
    The virus that causes COVID-19 attaches itself to cells in the upper airways when it enters the body. Inflammation sets off an immune response, leading to symptoms like cough, sore throat, and fever.

    Sometimes, the virus gets past the upper airway, moves through the lungs, and ends up in your alveoli. As a result, COVID-19 can lead to ARDS, which usually sets in 8 days after the initial symptoms. ARDS is more likely to develop in people with COVID-19 if they have advanced age, diabetes, or high blood pressure.

    Some COVID-19 patients are experiencing acute respiratory distress syndrome (ARDS) and require mechanical ventilation. While a patient is on a ventilator, Rush staff use a technique called “prone positioning” to improve oxygenation. Rush has developed a four-phase checklist to facilitate the process of placing a patient into the prone position.     The World health organization (WHO) has declared COVID-19 a pandemic. COVID 19 stands for coronavirus disease 2019 and is caused by the severe acute respiratory syndrome coronavirus 2.

    Coronaviruses are a large family of viruses that are common in people and many different species of animals including cats and bats. Common human coronavirus typically causes a URTI, like the common cold. Most people get infected with one or more of these viruses at some point in their lives. The human coronavirus infection typically resolves on its own with basic rest while feeling miserable.

    Rarely, the coronaviruses that infect animals can evolve and become a new human coronavirus which then infect and spread between people. Important examples of these type of coronavirus include severe acute respiratory syndrome coronavirus, SARS CoV for short in 2003 and Middle East respiratory syndrome coronavirus also known as MERS.

  • Coronavirus Update: Ventilatory Management for COVID-Related Respiratory Failure

    Management of COVID-19-related respiratory failure differs from what is necessary for ARDS. Rather than having alveolar edema, COVID-19 patients have pulmonary vascular dysregulation. Gas exchange is severely compromised with little reduction in lung compliance. Ventilatory support for COVID-19 patients requires higher than normal tidal volumes with minimal PEEP and allowance for higher than usual serum CO2 levels. How the unique pathophysiology of respiratory failure should be treated is discussed by John J. Marini, MD, professor of medicine at the University of Minnesota.

    In this lecture Professor Zach Murphy will present on COVID-19- Coronavirus: Epidemiology, Pathophysiology, Diagnostics.

    What is Corona virus? What is COVID-19? Coronaviruses (CoV) are a large family of viruses that cause illness ranging from the common cold to more severe diseases such as Middle East Respiratory Syndrome (MERS-CoV) and Severe Acute Respiratory Syndrome (SARS-CoV). Coronavirus disease (COVID-19) caused by SARS-COV2 is a new strain that was discovered in 2019 and has not been previously identified in humans.

    Coronaviruses are zoonotic, meaning they are transmitted between animals and people. Detailed investigations found that SARS-CoV was transmitted from civet cats to humans and MERS-CoV from camels to humans. Several known coronaviruses are circulating in animals that have not yet infected humans.

    Common signs of infection include respiratory symptoms, fever, cough, shortness of breath and breathing difficulties. In more severe cases, infection can cause pneumonia, severe acute respiratory syndrome, kidney failure and even death (WHO, 2020).

    Controversy surrounding the use of NSAID medications like ibuprofen and naproxen with COVID-19 infection have heightened now that the French government has issued warnings against them (Please Note: since the release of this video the WHO has walked back their warning about ibuprofen and coronavirus, and has said they are not aware of any negative effects. Dr. Seheult also discusses preliminary fatality data in the United States released by the CDC, and the results of a novel coronavirus treatment trial with HIV medications Lopinavir and Ritonavir in hospitalized COVID-19 patients.
